Parent Authorisation

I, the undersigned, declare that I will not hold The Arthritis Movement, its employees or volunteers responsible for any illness or injury to my child during Camp Footloose. In the event of any injury or illness, I authorise The Arthritis Movement staff to consent to emergency medical arrangements on my behalf, where it is impracticable to communicate with me, as is deemed necessary by a qualified medical practitioner. Such consent includes- anaesthetics, blood transfusions, and surgery and ambulance attendance. I will accept financial responsibility for any such action taken. I recognise that this is a camp for children and that it is important for the children to meet with others in the absence of their parents in order to develop rapport. I agree to leave my child in the care of the Camp Footloose staff and will not make arrangements to visit until the end of the camp. I understand that I can remove my child from camp at any time. I agree to sign the attendance register held by the Camp Footloose coordinator (when pick up is face to face) when I take my child home at the end of camp or beforehand, should he/she/they wish to leave if they are missing home or for any other reason. I will advise the camp coordinator or The Arthritis Movement if someone other than myself will be attending to pick up my child. I authorise The Arthritis Movement to require the signature of this person upon picking my child up.

We assure you that media materials will be used in a responsible and positive manner for the purpose of promoting the organisation’s activities and to secure funding for future events. Camp Guidelines

To ensure the safety and enjoyment of all campers we have included a behavioural contract for both you and your child to sign. It is an assumption that by signing this form your child will adhere to the following guidelines. Applications will not be accepted without this form signed by both child and parent/guardian.

CAMP FOOTLOOSE GUIDELINES

Be Kind and Respectful: Treat everyone with kindness and respect. Bullying, harassment, discrimination, or intimidating behaviour will not be tolerated.
Be inclusive: Help everyone feel welcome and included in activities and conversations.
No Visiting Other Cabins: Camper safety and privacy are of utmost importance. Campers are not allowed to enter any dorm room that is not their own.
Stay Within Camp Limits: Remain within the designated camp area and stay with your group unless given permission by camp staff.
Respect Nature: Dispose of waste properly, respect wildlife, and be considerate of the environment.
Camp Curfew: Quiet hours are from 10 pm to 7 am. Please respect these hours to ensure everyone gets a good night's rest.
No Vapes, Cigarettes, Alcohol, Drugs or Weapons: The possession or use of vapes, cigarettes, alcohol, illegal drugs and weapons (including pocket knives) is strictly prohibited.
Personal Belongings: Keep your belongings secure. The Arthritis Movement is not responsible for lost or damaged personal items. Campers must follow and adhere to the directions and requests of all staff members.
Room Tidiness: Keep your cabin tidy throughout your stay at camp and respect other camper’s belongings and personal space.
Participation in Activities: We encourage everyone to join in activities. If you choose not to participate, you must remain with the group and follow staff instructions.
Footwear Policy: Enclosed shoes must be worn at all times outside the cabin.
Tell a Staff Member if You Need Help: If you feel unwell, become injured, feel unsafe, or are worried about yourself or another participant, tell a staff member immediately. We are here to help you.
No electronic devices are permitted at camp. This includes mobile phones, tablets, gaming devices or similar. Campers are discouraged from bringing these devices, however they can be handed to group leaders for safekeeping on arrival. If otherwise found during camp, they will be confiscated and kept with group leaders. Campers can speak with their parents/Carers at any time via the camp leaders who carry a phone for this purpose.

DISCIPLINARY STEPS

The Arthritis Movement have sole discretion to determine whether your behaviour is in breach of the camp rules and will result in you being sent home.

Next Steps After Payment

After your payment is completed, you will be sent further documentation that must be completed before the relevant deadlines to secure your child’s place at camp.

1. Review the Camp Information Pack and Medical Information:
Please review the Camp Information Pack and Medical Information document for all the relevant camp details and submit a copy of the signed parent and camper acknowledgement.

2. Office of Sport and Recreation Registration:
You will also need to complete the online Office of Sport and Recreation Registration form. This is to advise the venue of the child’s emergency contact details, medical conditions, dietary requirements/allergies, and ensures compliance with their safety procedures.

3. Complete the Medication Chart and GP Medical Clearance Form:
Print the Medication Chart and the GP Medical Clearance form and take it to your child’s GP for completion. Once filled please send to [email protected].
Note: if your child does not require any medication during camp then they do not need to complete the medication chart. Every camper will need a GP Medical Clearance.

4. Prepare Medications for Camp
All medications (including those taken as needed or unrelated to arthritis) must be provided in a Webster Pack or they will not be accepted. Please read the Medical Information document for more details.

Restrictions on Injectable Medications
Injectable medications cannot be administered at camp. Please arrange for any necessary injections to be administered either before or after your child's time at camp.
